ameriCARE Fort Worth is proud to be among the first home care agencies in the area authorized to participate in the new GUIDE (Guiding an Improved Dementia Experience) Model, a pilot program launched by the Centers for Medicare & Medicaid Services (CMS) on July 1, 2024. This innovative model is the first of its kind to offer Medicare-covered respite care for individuals living with dementia. And we’re here to help local families make the most of it.

Understanding the GUIDE Model
What is GUIDE?

Caring for a loved one with dementia is a journey filled with emotional, physical and logistical challenges. Until recently, families navigating this journey have had limited options for support, especially when it comes to non-medical in-home care. That’s why the launch of the GUIDE Model marks such a significant shift in how dementia care is delivered and supported across the country.

This program introduces a first-of-its-kind opportunity for individuals with dementia and their families to access a comprehensive range of services, many of which are now covered by traditional Medicare. By addressing both clinical needs and social support, the GUIDE Model ensures a more holistic approach to dementia care that prioritizes the well-being of both patients and caregivers.

The program aims to:

  • Improve quality of life for people living with dementia
  • Reduce stress and burnout for unpaid caregivers
  • Help individuals remain safely in their homes longer
  • Minimize unnecessary emergency room visits and hospitalizations

Through a combination of 24/7 support lines, personalized care coordination, caregiver education and Medicare-covered respite services, the GUIDE Model offers families a lifeline, providing meaningful and practical help often for the first time.

ameriCARE Fort Worth contracts these services through Healing Hands, a locally owned and operated home health provider. 

Eligibility Who Qualifies for GUIDE?         

While each situation is unique and final eligibility depends on specific criteria, the general qualifications for GUIDE participation include:

  • A confirmed diagnosis of dementia
  • Enrollment in traditional Medicare (Part B)
  • The individual is receiving in-home, community-based care (not living in a nursing home or other institutional setting)
  • The presence of an unpaid caregiver, such as a family member or friend, who provides regular assistance and support
